QA Personnel Responsibilities and Expectations
On-Site QA Verifications

Perform systematic inspections against project-specific criteria covering construction quality, labeling, safety, test results, documentation, and risk identification.

Log hazards and recurring issues, escalating per project timelines and reporting standards.
Photo Documentation Review

Organize, review, and tag contractor photos for compliance and completeness (before/after shots, required angles, inclusive of geo-tagged and timestamped data).

Provide pass/fail determinations and ensure photo sets support as-builts and redlines.

Validate fiber certification records and confirm test results are within compliance, with fiber assignments aligned to approved designs and redlines.
Defect Tracking and QA/QC Record Keeping

Maintain a QA/QC log linking inspections, street sheet references, redlines, and supporting evidence.

Manage defect tracking through to closure: assign responsibility, verify remediation in the field (including photo and record verification), update the log to “resolved,” and escalate any unresolved or recurring issues with a formal risk report.
Field Validation for Construction Payment (OSP, Drops, Installs, Pre-Wire)

Verify routing, protection, bend radius, clearances, attachments, labeling, and terminations for all work typesâincluding outside plant (OSP), service drops, customer installs, and MDU pre-wire.

Identify and document quality, clearance, or safety concerns for correction prior to payment approval.
Invoice / Sign-Off Support

Issue QA sign-offs used to validate vendor invoices, contingent upon passing inspections, street sheet validation, and as-built/redline compliance.
Coaching and Governance

Provide field feedback, micro-training, and recurring defect and risk analysis.

Drive corrective actions and ensure vendor onboarding and workmanship gaps are addressed.
Closeout

Confirm all corrective actions are complete, deliver final as-builts/redlines, and obtain final QA sign-off.
Staffing and Field Presence

Provide qualified QA personnel for planned work windows; participate in ride-alongs and spot checks as needed.

Personnel must be experienced in OSP construction, fiber testing, and safety practices.
Safety and Clearances

Verify compliance with required safety practices and clearance standards.

Document and report any deviations with associated risk notes.

Ensure all prime and subcontractor personnel complete required background checks and drug screenings prior to site assignment.
Contractor-Furnished Equipment, Vehicles, and Communications

Vehicle: Licensed and insured; equipped with amber beacon, reflective triangles, and traffic cones.

Test & Measurement Tools: OTDR, optical power meter, visual fault locator (VFL).

General Tools: Screwdrivers, pliers, cutters, telecom hand tools (including can wrench), fiber prep tools (stripper, cleaver, alcohol wipes), manhole/lid keys, probe rod, hand shovels, and indoor-rated ladders with non-marking feet.

PPE & Safety Gear: Class 2 high-visibility vest, hard hat, safety glasses (clear/tinted), cut-resistant gloves, work gloves, and safety footwear.

Communications & IT: Mobile phone with data/hotspot capability and a ruggedized tablet or laptop.
Telematics / GPS Tracking

Utilize GPS-enabled devices or applications to document field presence, route efficiency, and compliance with assigned work locations.
